Purpose: To evaluate tear neuropeptides (NPs) – VIP, NPY, CGRP, SP, NGF in chronic ocular
topical hypotensive therapy with & without BAK preservative
Methods: Prospective comparative study of eyes on chronic topical antiglaucoma therapy with (group 1) & without BAK (group 2) & controls for tear NPs, TBUT, Schirmer’s, staining score & corneal subbasal nerve fibre layer density (SBNFLD)
Results: Group 1 (82), Group 2 (71), mean age 48±14.5, 43.11±15, mean therapy duration of 10.05±2, 9.67 ± 2.3 months respectively had elevated SP, NGF (p=<0.01), decreased CGRP (p=0.03), VIP, NPY (p<0.01) compared to controls (30, mean age 29.33±5.7 yrs). Conclusion: With level 2 DED in BAK preservative & level 1 DED in BAK free eyes, tear NPs (SP, NGF) were noted to be elevated significantly; CGRP, VIP, NPY significantly decreased in chronic topical ocular hypotensive therapy (both groups) compared to controls. No significant difference in tear NPs & SBNFLD between the BAK preservative & BAK free eyes
