Purpose of this study was to evaluate preventative surgical options for negative dysphotopsia.5 patients of self reported chronic negative dysphotopsia were included in this retrospective study.All patients had corrective surgery as the therapeutic group.Second eye surgery in cases with negative dysphotopsia in the previously operarted eye comprised the preventative group.Several surgical options were used including bag to bag IOL exchange,reducing posterior chamber depth,piggy back secondary IOL placement,bag to bag sulcus IOL exchange and reverse optic capture.Improvement of negative dysphotopsia was recorded 3 months postoperatively.Negative dysphotopsia was associated with acrylic or silicon IOL of either square or round edge design.It was reduced and eliminated when the IOL optic overlayed the anterior capsulotomy rather than when the capsule edge overlayed the optic.Bag to sulcus IOL exchange and reverse optic capture were highly successful.
