Swan syndrome is focal vascularization from an ingrowth of episcleral vessels at the cataract wound site, resulting in recurrent intraocular bleeding. It has been reported after ICCE,ECCE,glaucoma filtering surgeries.
A 40 yr male, high myope with uncontrolled POAG in LE underwent an uneventful Phaco+ IOL following which.he had recurrent uveitis which responded to steroids
After 3yrs Patient came with DOV LE.No h/o trauma,with Small Hyphema,which resolved in a few days,BCVA, A/S, disc , retina and IOP were normal
Gonioscopy: Showed a pinkish blush on the internal ostium ? neovascularisation and the
vessels were seen to actively bleed with pressure from the Goniolens. angle was open
with moderate pigmentation. A diagnosis of SWAN syndrome was made.
SWAN syndrome is rare , but needs to be kept in mind as a DD for recurrent inflammation with
hyphema after anterior segment surgeries, with a scleral approach. Anti VEGF drugs can be
effectively used in the management of this condition.
