We present details of 2 rare cases of hypotropic DVD (HDVD) in which eye drifted slowly downwards instead of upwards during period of inattention. 1st case is a 21 year old man who underwent RD surgery with inferior buckle in situ. He presented with downward drift of left eye (LE) during periods of inattention which drifted back during attention. It was a manifest HDVD measuring 35 pd by PBUCT. He underwent LE Inferior Rectus 20mm Y split with recession 7mm. His HDVD got eliminated till 1 year follow up. 2nd patient was a 14 year old boy who presented with RE 10pd DHD with RE HDVD and LE 20pd DHD with HDVD. On putting Speilman occluder on RE, it could be seen slowly drifting upward which drifted down on removing it. But on putting Speilman on LE, it drifted outwards but after few seconds it slowly drifted down which on removing occluder slowly drifted to primary position. Since patient was not willing for surgery & his HDVD was latent, no intervention was done & kept under follow up.
