Introduction- Retinal pigment epithelial detachment (PED) is a part of age-related macular degeneration clinical spectrum. Different types of PED have been reported and they have been either related or not with AMD or CNVM. Their treatment is always dicey. We present here a similar case of PED. Method- A middle aged lady presented with significant reduction in vision of one eye since a week. She was found to have a large subfoveal PED. Investigations did not reveal any clear evidence of CNVM. Since it was a large PED (horizontal in shape) and completely obscuring fovea with significant drop in vision, intravitreal anti VEGF injection was advised with explained prognosis. Result-At the end of first month itself and subsequently, the patient recovered well both anatomically and functionally. The PED reduced considerably in size. Conclusion- In PED s of unknown causes with horizontal morphology anti vegf can be tried. Care should be taken in vertical and tight PED s to avoid rip
