Spontaneous corneal perforation in PMD is rare and has no clear cut management protocols even though multiple treatment strategies have been described in literature with variable outcomes. We report a case of spontaneous unilateral corneal perforation in a 64 year old male patient with normal corneal thickness which was managed with primary suturing and compression sutures which helped not only in the acute management of the disease but improvement of the best corrected visual acuity (BCVA) by topographic regularization. Use of this novel treatment modality could also help to do away with traditional methods such as fibrin glue and corneal transplants which have certain other complications
