A 65 yr old female presented with a pigmented lesion on right lower lid for the past 2 yrs which had gradually increased in size in last 6 months.On examination, the lesion involved medial half of lower lid measuring 32mm x 27mm sparing the punctum and lid margin.The surface of the tumour was excoriated and fixed with the underlying structures.Excision of the carcinoma was performed leaving 3mm tumour free margins. Raw area was reconstructed by adjacent skin from temporal area by Tenzel semi-circular flap technique.The biopsy report confirmed it to be pigmented BCC. Post-operatively the patient gained good cosmesis without any complication and recurrence.The reconstruction technique should be tailored according to the patient and location of the tumour. Local flaps being easy to harvest and handle, provide good results due to good matching of the skin in terms of colour, texture, and thickness. Detailed surgical steps alongwith the postop cosmesis achieved will be discussed in detail.
