Purpose: To emphasize the role of slit lamp bio-microscopy and Specular Microscopy in keratoconus cases. Methods: 2 cases whose visual acuity and keratoconus stage, thickness of the cornea on peentacam scans was not co-relating when compered to the fellow eye under went Specular Microscopy. Results: Specular Microscopy of the two cases helped in solving the discrepancy of visual acuity and keratoconus stage. Conclusion: All keratoconus cases must under go detailed slit lamp bio-microscopy examination to examine the endothelial layer if discrepancies are noted between the stage of keratoconus, thickness of cornea on pentacam scan and visual acuity and when compared with the fellow eye a Specular Microscopy is warranted.
