A 42year old woman admitted as orbital cellulitis with 10 days history of left ocular pain, swelling, fever. Clinical and Lab examinations showed lid edema, conjunctival chemosis and injection with leukocytosis, increased ESR and CRP. She responded to IV antibiotics. IM corticosteroid started on 5th day for 4 days only. All symptoms disappeared but reappeared after 2 days. MRI revealed inflammatory changes in lacrimal gland and extraocular muscles. Diagnosis was changed to orbital pseudotumor. Antibiotics discontinued, IM steroid restarted. She responded.
