A 25 year old female developed chemosis of eyes following MTP of missed abortion.She had fever and UTI 1 week prior.On examination vision-1/60,chemosis,EOM-restricted in all gazes,fundus-established disc oedema.MRI BRAIN and LP-inconclusive.COUNTS-normal.Blood/urine/csf culture no growth.Serology reports for infections-negative. Patient started on IV antibiotics,LMW Heparin,Diamox and oral steroids in view of clinical suspicion of cavernous sinus thrombosis,optic neuritis and raised ICT.APLA and ANA-positive.VEP-bilateral anterior visual pathway conductive defect.Repeat MRI-normal.On review vision Right eye-6/18with PHNI,Left eye-CFat2M with PHNI,chemosis had decreased.Left eye showed exotropia,RAPD,medial movements restricted.Patient put on oral and topical antibiotics,oral anticoagulants and glasses prescribed.On further follow ups patient vision improved Right eye-6/6.Left eye-6/9 with strain.Thus decisive and timely treatment has a rewarding outcome for the patient and doctor.
