A 45 year old female patient presented with an avulsion injury of the right side of the face following a road traffic accident. Intraoperatively, the medial canthal ligament was found to be torn off and upon injecting methylene blue dye through the lower punctum which was intact the lacrimal sac was stained blue. Bi-canalicular stenting of the two canaliculi going into the sac was done and the lacrimal sac was repaired. Medial canthal re-fixation was done before closing the skin wounds in layers. The eyelid tissues placed back to their anatomical positions before suturing the skin lacerations in layers. Postoperatively she was treated with 5fu and steroid injections for scar release sofenting.a z plasty was done for proper positioning of the medial canthus and to achieve good cosmesis.
