43year male presented with diminution of vision in left eye, on recurrent treatment with topical and systemic steroids for uveitis in multiple institute for 8 years. R/E eviscerated following penetrating ocular injury. O/E LE vision was HMCF. Slit lamp revealed flare1+, cells 2+, old keratic precipitates, seclusive pupillae, typical iris coloboma and cataractous lens. Fundus could not be evaluated. Depigmented lesion seen over both eyelids and face with poliosis.
Moderate degree of sensorineural hearing loss with no other neurological deficit noted. Normal lab investigations and B scan showed vitreous debris and attached retina. Main differentiating feature being penetrating ocular injury in the other eye, this case was diagnosed as sympathetic ophthalmitis.
Under cover of oral steroids, topical antibiotics, antiglaucoma, patient underwent cataract surgery with PCIOL in left eye. Patient was on follow up with topical corticosteroid and immunosuppressant. Vision improved to 6/12.
